About Cagrilintide

Cagrilintide is a long-acting, lipidated analogue of the pancreatic hormone amylin, developed by Novo Nordisk with obesity as its indication ^[1]. It appears in the literature under the development code AM833 ^[2], its combination with the GLP-1 analogue semaglutide is known as CagriSema ^[1], and it is often called Cagri for short.

Development history

Amylin readily forms amyloid fibrils, which makes it a difficult starting point for drug design, and pramlintide, the analogue already available for diabetes treatment, has a short half-life ^[1]. Kruse and colleagues described the structure-activity work behind cagrilintide, a stable analogue whose peptide carries an attached fatty-acid chain ^[1]. In a phase 1b trial alongside semaglutide 2.4 mg, cagrilintide had a half-life of 159 to 195 hours across the doses studied ^[3].

Mechanism of action

Natural amylin is a pancreatic hormone that induces satiety ^[4]. Its receptors are heterodimers of the calcitonin receptor with receptor activity-modifying proteins (RAMPs), and cagrilintide is a nonselective agonist, acting at these amylin receptors and at the calcitonin receptor on its own ^[2]. Profiled across 25 endpoints against six comparators, including pramlintide and salmon calcitonin, it showed a unique profile of receptor binding, activation and regulation ^[2].

Monotherapy trial

A 26-week, dose-finding phase 2 trial at 57 sites randomised adults with overweight or obesity and without diabetes to once-weekly cagrilintide (0.3 to 4.5 mg), once-daily liraglutide 3.0 mg or placebo ^[4]. Mean body weight fell by 6.0% to 10.8% across cagrilintide doses against 3.0% with placebo, and by 10.8% at 4.5 mg against 9.0% with liraglutide ^[4]. Gastrointestinal adverse events, chiefly nausea, occurred in 41% to 63% of the cagrilintide groups against 32% with placebo ^[4].

Combination research

In a 32-week phase 2 trial in 92 adults with type 2 diabetes, mean HbA1c fell by 2.2 percentage points with CagriSema, 1.8 with semaglutide and 0.9 with cagrilintide (significant against cagrilintide, not semaglutide), and body weight by 15.6%, 5.1% and 8.1% ^[5]. The phase 3 REDEFINE trials ran for 68 weeks with 2.4 mg of each component ^[6,7]. In REDEFINE 1, among 3,417 adults with overweight or obesity and without diabetes, estimated mean body weight fell by 20.4% with CagriSema against 3.0% with placebo ^[6]. In REDEFINE 2, among 1,206 adults with type 2 diabetes, it fell by 13.7% against 3.4% ^[7]. Gastrointestinal adverse events were reported in 79.6% and 72.5% of the two CagriSema groups against 39.9% and 34.4% with placebo ^[6,7].

Research areas and status

Published work uses cagrilintide to study amylin and calcitonin receptor pharmacology ^[2], body weight regulation ^[4], glycaemic control in type 2 diabetes ^[5] and how amylin receptor agonism may act additively with GLP-1 receptor agonists ^[2]. As of October 2026 it remains investigational; neither cagrilintide nor CagriSema holds a marketing authorisation from the MHRA or the FDA.
